Casualty: BBC teases first-look at 40th anniversary 'landmark' double bill about a 'high-stakes major incident'

BBC One has unveiled the first-look at Casualty’s 40th anniversary double-bill, and it looks as nail-biting as you’d expect from such a landmark episode.

In 1986, the medical drama first landed on our screens, introducing characters we grew to love. Charlie Fairhead, Lisa ‘Duffy’ Duffin, Barbara ‘Baz’ Wilder, and Megan Roach we’re talking about you (RIP Brenda Fricker).

Forty years later, it’s only fitting that the series marks its special birthday with an event-TV episode. And the Beeb isn’t planning to disappoint its loyal fans.

So what can we expect from the double-bill, airing on Saturday, September 05, 2026 on BBC iPlayer and BBC One? Here’s everything we know so far.

Casualty celebrates its 40th anniversary with a double bill

Casualty is the world’s longest-running medical drama, having been on our screens for four decades. During that time, it has delivered powerful storytelling, unforgettable characters and ambitious television moments. And that’s not about to stop now.

To celebrate 40 years on the Beeb, the broadcaster is planning the mother of all double bills. Fans will know that the drama has marked each significant anniversary with a special episode. For the 30th anniversary in 2016, the feature-length 100-minute episode – entitled Too Old For This Shift – focused on Charlie Fairhead’s 30 years in the NHS amidst a devastating drone crash hitting an air ambulance.

Meanwhile, the 35th anniversary in 2021 began with a feature-length special focusing on the harrowing realities of the COVID-19 pandemic on emergency staff, alongside the return of popular past characters.

What happens in the special episode?

The gripping, anniversary event centres on a “high-stakes major incident that throws Holby into chaos”. We’re told that “lives hang in the balance” as the ED team are pushed to their limits in a tense and emotionally charged story. The chilling trailer shows a huge explosion, followed by red alerts flashing across the county of Wyvern, as multiple bodies are rushed into the ED.

The gripping double-bill will bring together some of the show’s most beloved characters. The episode will star:

The BBC teased: “As the fallout continues, a major rescue operation unfolds, forcing impossible life-or-death decisions and testing the courage, resilience and leadership of those at the heart of the hospital. As Dylan battles personal tragedy while leading his team through their darkest hour, Stevie and Flynn find themselves fighting for survival in a race against time.”

The Beeb has also revealed that the anniversary episodes “signal an exciting new chapter” for the medical drama.
